Position Title: Social Work, Care Manager

Position location: Wake County Medical Society Community Health Foundation; Community Care of Wake and Johnston Counties- position is primarily home based with regular travel varying from 25-75% of the work week within Wake and Johnston Counties

Position Summary

Purpose: Community Care of Wake and Johnston Counties (CCWJC) is one of 14 local networks providing care management across the state. The program is sponsored by the Division of Medical Assistance (DMA) and the Office of Rural Health and Community Care. Care Managers work with local providers to develop community-based managed care networks and infrastructure to meet the health care needs of the Medicaid population, while providing the State with greater budget predictability.

Summary of Position: The primary purpose of the Care Manager is to address the programmatic and preventive needs of the Medicaid Carolina Access population by assessing, planning implementing, coordinating, monitoring and evaluating the options and services required, and using communication and available resources to promote quality, cost-effective outcomes.

Position Summary

Purpose:
Woody’s Mom, Inc., a CAP-C Case Management Agency, has two open positions due to program growth.

Summary of Position:
Registered Nurse Case Managers serve an average caseload of 30-40 medically fragile children who live at home. We write assessments and care plans, make referrals to community resources, continuously monitor services for quality and utilization, and collaborate with members of the child’s clinical service team to assist in reaching personal goals.
